NHS Wales Exam Types and Risk Assessments (ACORN)
On 1st April 2022, NHS Wales introduced changes to the information required on FP17's and how ACORN assessments were completed.

Requirement of a Risk Assessment
- Patients require a Risk Assessment to be completed once within a 12 month time frame, or at the time of a patients dental recall, if set longer than 12 months.
- One ACORN is required per patient within a 12 month date range, any treatment outside of this does not require an ACORN Assessment.
- For patients where an ACORN could not be completed or one is not due, should have their FP17 marked as 'Interim Care'.
Examination Types on the FP17
Please Note: The Exam Type should match the charting on the Course of Treatment (COT). For example there should not be a completed Risk Assessment with the Exam Type of Interim Care Review on the FP17. |
Acorn Assessment Carried Out
- This should be set on the FP17 for any treatment where a Risk Assessment was completed and present on the Course of Treatment (COT).
Exam Not Possible
- For use where an examination was not possible.
Interim Care Review
- Only to be used where no Risk Assessment was completed as one has already been completed within the last 12 months.
Exam Not Necessary
- To be used when no examination was required.
Urgent Care
Emergency treatment no longer requires an ACORN Assessment to be completed. As EXACT automatically adds the Risk Assessment to a Course of Treatment (COT) you will need to delete the Risk Assessment from the COT then chart all treatment as normal.
Configuring the NHS Risk Assessment
The NHS Wales Payor has a revised tick box on Payor File: “Auto chart NHS Risk Service on COT creation”. It is on by default and will automatically add “NHS Risk Assessment” service to planned UDA COTs if a patient has not been assessed by the same provider in the previous 12 months.
An NHS Risk Assessment will NOT be automatically added to a NHS UOA (orthodontic) COT:
Viewing a Risk Assessment
This can also be actioned via the Tooth History.
- Click the history button H on the chart tab.
- Locate the Risk Assessment service, then double-click on the row.
- Select View Risk Assessment.
- Click between the tabs to view.
Warnings
There are several warning messages that may appear in conjunction with the completion of a Risk Assessment.
Unticked Risk Assessment Treatment Item
Warning if ticking Course of Treatment complete without completing the ‘NHS Risk Assessment’ service:
Incomplete Risk Assessment
Warning if ticking Service below - Incomplete Risk Assessment dialog appears with missing sections Relevant History, Clinical Findings and Perio Health.
The tab with missing data will be listed in the prompt to highlight where you need to add extra information.
