Medical Histories and the ACORN Risk Assessment in EXACT are linked, with information collected on a medical history added to the Behaviours and Protective Factors tab depending on what Medical History form you have configured.

With the Fixed Medical History Advanced form enabled, if you have Patient Portal you will be using this version all of the relevant information will be cross-populated.

For example, if the option Lots of fizzy/acidic drinks is ticked on the patients Medical History, this will be ticked on the Risk Assessment.
