Performing a Denplan Assessment ensures you put your patient on the correct Denplan plan and be charged the appropriate fee.
Denplan Assessments
⚠️ Please Note: Denplan functionality must be enabled and configured to access Denplan assessments.
💡Tip: If your assessments are producing scores of 0 (zero) and you have only recently configured Denplan in your system, log out and back into EXACT.
- Load your patient record.
- Check or set the Payor to DENPLAN.
- If you set this, click the save icon at the top of the patient record.
- If you set this, click the save icon at the top of the patient record.
- Select the Denplan tab.
- Then select the Assessments tab.
- Existing patient charting will be used to populate the assessment.
- Planned treatment will not be included.
Performing a Denplan Assessment
💡Tip: Previous assessments will be listed in the Assessments tab.
- Click the +1 button to create a new assessment.
- The Add Denplan Assessment window will open
- Existing patient charting will be used to populate the assessment with Filled Surfaces, Root Filled Teeth and Prostheses Worn data.
- Planned treatment will not be included.
- A score will be calculated using this charting and Denplan assessment scores.
- This data represents the scoring value, not the number of amalgams or composites a patient has received. For example, a Composite has a scoring equal to 2, and a patient has had 3 composites, so the score is 6.
- You can edit these scores, as these are guidelines which will update the subtotal entries.
- Manually updating a score in the form:
- Click in the field to display a numerical control for that field, then use the Up or Down arrows to change the value. The subtotal score and total score will update.
- The Date defaults to today's date, but can be overwritten if required.
- Choose the appropriate Provider.
- You will need to manually set values for Gingival Condition, Periodontal Status, and Plaque Control sections.
- These items will be set to zero subtotals when the assessment is first opened for you to enter an appropriate selection using the drop-down list, if the default conditions are not correct for the patient.
- The Gingival Condition, Periodontal Status, and Plaque Control sections of the DAF can be manually changed to any number you wish.
- As you enter the figures, EXACT will automatically subtotal and total the scores.
- The Proposed Band field will update to reflect the total assessment.
- When you are satisfied that the assessment is complete, click OK.
- The Add/Edit OHS prompt will now open, this contains information identifying the patient's current Denplan band (before the latest assessment).
- Their new banding, as per the assessment scoring.
- You also have the ability to manually set the patient's band, using the dropdown menu.
- Click the Update Patient option to to update the patient's record, or click the Leave Patient option to leave the patient with their original band.
- Once updated, the assessment will be added to the list in the Assessments tab.
Denplan Assessment Scores
The scores on the Denplan Assessment screen are the same as those on the printed Denplan Care Assessment form (see attached form at the bottom of this article).
The following tables show the scores for each of the categories in the on-screen Denplan assessment form.
Scores for a service item are changed by selecting a Denplan category for that service. On the final assessment the calculated score can be overridden by simply typing in a new score.

Additional Information
💡Tip: If your assessment is not auto-scoring for some services, check these have a Denplan category assigned to them. Go to Configure - Services (or click the e|d button on the Chart screen). Once you have the Edit Service screen open, select the appropriate Denplan Category from the available list.
