NHS FP17 form
An FP17 is sent to the NHS electronically via EXACT as part of every NHS claim. This contains various information relating to the claim and can be check at any point via either,
- Go to Administration - NHS - Course of Treatment and enter the COT number in the bottom left hand corner then double click on the claim.
or - Go to the patients file and click on the £ on the toolbar then single click on the COT.
- Click the FP17 button to view.
NOTE: It is recommended that both the Show Open Treatment and Show Resubmissions check boxes are not ticked, as transactions with either of these statuses are not associated with checking the FP17/GP17 form. In turn, reducing the list of transactions displayed. |

The FP17 form
The form will appear as below or very similar, depending on the version of EXACT, we advise being on the latest version to adhear to all NHS regulations.

Welsh practices will have two drop-down menus for Exam Type and Patient Presented with for compliance with new rules introduced on 1st April 2022.
These menus will allow for you to choose both the Exam Type and why the patient attended your dental practice.

Recall Interval (9172) and NICE Guidance
Patients' recall intervals are now transmitted with NHS claims, using the following information.
The recall interval information as displayed on the Recalls tab on a patient file will be added to the FP17 window. Updating a patient's recall interval on either the recall tab or the FP17 will update the other. NICE Guidance has been updated to represent this update.
To alter a patient Recall Interval on the FP17 window, use the drop-down menu in the upper right to select the correct interval, this will update in all areas of EXACT.
UDA/UOA Transaction Summary
To view the UDA/UOA transaction details for claims and UDA management,
- Click the Details button to the right of the UDA information.
- This screen shows all the claimed and confirmed transactions for the current COT.
- When you have reviewed them, click the OK button to close the screen.
- Click the Close button to return to the Payor Transactions window.
- If errors are found within a form it can then be held for later correction. NHS England, Wales & NI - Holding Claims
Exemptions/Charges
This will display any patient exemptions entered as per the exemptions window upon opening a course of treatment or via the Exemption button at the bottom of a course of treatment and the patients age.
Patient Consent
Patient consent for Contact Preference is auto-populated with a patients information as entered via Clinipad and Patient Portal. The default is to opt-in to be contacted by the NHS, patients can still choose to opt-out by removing the tick from the box next to the email address or mobile number.
Note: If you do not use Patient Portal or Clinpad this information will need to be manually entered. This is to eliminate the chance of a possible Data Breach, supplying patients information to the NHS without consent. In this instance, please enter this information manually. |
Treatment
This will display the selected banding for the treatment, any treatments exempt from charging and the AGP appointment counter.
Clinical Data Set
All data sets assigned to charted treatments will be displayed on this window, set as per NHS Datasets, Service Codes & Categories.
DMF - Decayed, Missing & Filled teeth
This tab will display a counter for all missing, decayed or filled teeth and can be edited. This also displays the highest BPE sextant score.
Number of Untreated Decayed Teeth (9379)
The number of untreated decayed teeth is required to be sent with all courses of treatment, the default setting is 0 which unless changed by the practice will be transmitted with the claim to the NHS.
Set the No. of untreated decayed teeth on the NHS FP17 by entering the total number in the box on the DMF tab.
Highest BPE Sextant Score (9378)
The Highest recorded BPE sextant score will be transmitted on all NHS claims from 1st October 2022.
The Highest BPE score is set according to the BPE scoring on the patient file or charting and can be edited on the FP17 window. This is then displayed on the DMF tab on the NHS FP17 after TC'ing a course of treatment.
Ortho
The orthodontic tab will display all information relevant to the orthodontic treatment, the UDA Summary section will display UOAs when charting treatment on an UOA cot.
