Troubleshooting FP17 Forms not linking to Treatment
If an FP17 form has been completed online using Patient Portal but has not been linked to the Course of Treatment (COT), this is often caused by an incorrectly completed FP17 form.
💡Tip: Check in the Patients Contacts tab for the FP17 form, if it's stored in there, it's likely that something has been completed incorrectly.
FP17 Forms not Linking for Patients
The most common issue is that the incorrect exemption has been selected when completing an FP17 form online via Patient Portal.
FP17 Forms for Children not Linked
This issue is often caused by the exemption Do not pay as under 25/over 60 has been selected.
In this instance, the FP17 will be saved to the patient Contact tab, with an identifier showing that the form has been incorrectly completed.
To correct this, the patient must complete a new FP17 form, selecting the Under 18 exemption.
FP17 Forms with Pregnant Exemption Selected, not Linked
This issue is often caused by an incorrect mix of sex being male and the Pregnancy exemption being selected.
To correct this, the patient must complete a new FP17 form, selecting the correct exemptions, eg Pregnancy if female or not pregnant, if male.
