Medical History Configuration
The configuration of the Medical History forms used in your practice can be updated at any point by a user with security access.

Medical History Configuration
To update the practice Medical History settings:
- Go to Configure - Appointment Book.
- Select Arrival Options then click Medical History.
- This can also be access via the spanner on the Medical tab of any patient.
- This will open the configuration options, this window gives you access to setup;
- Setup which Medical History Form being used, Custom or Fixed (Patient Portal and Clinipad require the Fixed Advanced form to be used).
- Set how often a Medical History is requested via Alert if Medical History is overdue.
- Whether the Medical History Alert flashes and whether it is appears whether it appears when you move patient to the chair.
- If you have CliniPad configuration can be set for the Medical History to be completed in surgery and whether the dentist signs it.
- An option is also available to Request a new Medical History for all patients.
Please Note: If you edit or change the Medical History form being used, every patient will be required to complete a new Medical History from scratch, with any information from previous Medical Histories not being carried over to the new form.
Reverting the Medical History form back to a previously used Fixed option will pre-populate the patient's previous answers if their last Medical History used the same version of the form.
Fixed Medical History Forms

Fixed forms contain a pre-determined set of questions that cannot be altered, these are often created in-line with your countries rules and regulations as outlined for standardised medical questionnaires. This standardisation lends itself well to the use of forms for Digital use, such as Patient Portal and Clinipad.
There are three options to select from when using Fixed forms;

- Simple, is a series of basic questions, this form helps to reduce the number of questions a patient is required to complete.
- Default, contains all those on the simple form with the addition of a number of symptoms and a Habits section to collate additional information regarding smoking, alcohol, fizzy and recreational drugs.
- Advanced forms contain a full list of options and improved selection options for yes/no or blank for neither.

Custom Medical History Forms

Custom forms can be edited as per your practices requirements. These are created from Custom screens, your EXACT comes with some Medical History custom screens as default, however, you can customise these or create a new custom screen.
To select a Custom Medical History;
- When in the Medical History Configuration, select Custom Medical History.
- Then click the selection option to the right of the Custom Screen drop down menu.
- Select an option from the list then click OK to add to your Medical History configuration.
Setting the Frequency the Medical History is Taken (Medical History Alert)
You can decide how often a Medical History is taken by changing the Alert if Medical History is X months overdue setting.
The number of months you set will determine how long after the last Medical History is taken that EXACT will prompt users to update it.
Tip: Set this to 0, to stop any prompts reminding you to check patients Medical history, regardless of how long it was since it was last updated. |
User Settings
This section of the Medical History configuration is per user, allowing for different configurations for each user in your practice, you can configure the following options;
- MED Alert Flashing sets if the Medical History Alert icon will flash.
- Open Medical History on 'Move to Chair' is for providers.
- When using Clinipads, it can be configured to Have patients sign their Medical History in surgery if a provider wishes to do this themselves.
- In addition to the above setting, it can also be configured for the Dentist to counter-sign the form after reviewing it with the patient using Require dentist signature.
- By default the Receptionist will be prompted at patient arrival to complete a medical history form, but this can be disabled so that it is not prompted.
