Medical History Configuration
Patient medical history can be configured to suit the requirements of your practice.
Patient Medical History Forms
⚠️Please Note: If you change your default template, all patients will be required to complete a medical history the next time they attend your practice, regardless of whether they have recently completed a form.
⚠️Please Note: Clinipad and Patient Portal require pre-configured medical history forms; custom forms can not be used if your practice utilises either of these features.
- Go to Configure - Appointment Book.
- Select Arrival Options, then click Medical History.
- This will open your Medical History Configuration options.
- You can select between Custom Medical History and Fixed Medical History
⚠️Please Note: Only fixed medical history forms can be used with Patient Portal and Clinipad.- Selecting Custom Medical History enables access to all available custom screens.
- Selecting Fixed Medical History enables the dropdown menu for Simple, Default and Advanced options.
- An option to Request new Med Hist for all patients is also available; selecting this will reset the expiry date for all patients' medical histories.
- The Alert if Medical History is overdue setting is used to configure warnings if medical histories are out of date.
- The final section can be configured per provider, use the dropdown menu to edit individual provider settings.
- Med Alert Flashing set if the alert will flash or not.
- Open Medical History on 'Move to Chair' will set if this happens or if it must be opened manually.
- Whether the medical history Alert flashes and whether it is appears whether it appears when you move patient to the chair.
- If you have CliniPad configuration can be set for the medical history to be completed in surgery and whether the dentist signs it.
- An option is also available to Request a new Medical History for all patients.
- You can select between Custom Medical History and Fixed Medical History
Fixed Medical History Forms
Pre-configured or fixed forms cannot be altered or amended. These are often created in line with your country's rules and regulations for standardised medical questionnaires. This standardisation is essential for digital forms, such as Patient Portal and Clinipad.
There are three options available.
- Simple, consisting of a series of basic questions to keep things simple.
- Default, the same as simple, with some additional symptoms and habits to collate additional information regarding smoking, alcohol, fizzy and recreational drugs.
- Advanced, contains a full list of options with improved selection options.

Custom Medical History Forms
⚠️Please Note: Custom Medical History forms cannot be used if your practices uses Clinipad or Patient Portal.
Custom forms can be edited as per your practice requirements. These are created from custom screens, your system is preloaded with a selection of custom medical history screens as default, however, you can customise these or create a new custom screen.
To select a custom medical history screen.
- Select Custom Medical History in the Medical History Configuration.
- Click the selection option to the right of the custom screen drop down menu.
- Choose an option from the list.
- Click OK to select this custom screen.
Additional Information
💡Tip: Set the frequency to 0, to stop any prompts reminding you to check patients medical history, regardless of how long it was since it was last updated.
