The NHS FP17DC or FP17DCO form is a treatment plan that NHS dental practices provide to patients before a course of treatment (COT).

Creating an FP17DC or FP17DCO

1. Open an NHS course of treatment.
2. Chart all treatments to be completed.
3. Create an estimate of the COT.
   • Follow on-screen prompts, selecting the options as required.
4. You will be prompted to create an estimate.
   • Select all options as required.
   • You can preview your choices using the preview option at the bottom of the prompt.
   • Once complete, select print or email to produce a copy for your patient.
   • A copy will be saved to the estimates tab on the patient record.
