Manually Request Patient Forms with Patient Portal
Patient forms can be manually requested from patients and sent via Patient Portal.
💡 Tip: We advise using automation to request forms from patients via Patient Portal, as this helps eliminate the replication of forms displayed for patients to complete and reduces confusion for patients.
🔗 Sending Forms to Patients using Patient Portal
Send Forms Manually with Patient Portal
- Click Patients and then open the patient record.
- Click the Patient Portal button along the top of the patient record window (Cloud icon).
- Select the forms you wish to request manually.
- Click Close when all chosen forms are requested.
- Click the Email or SMS button along the top of the patient record window.
- Select the relevant Portal communication to be sent.
⚠️Note: This communication must contain the [patient.portalURL] merge field, as this will add a link for the patient to access their forms online. - Click Send.
- Select the relevant Portal communication to be sent.
- The patient will now receive a communication from you with a link to access all the forms that have been requested.
