You must use the 'Advanced' Medical History form', if you are using Default and need to switch, the data will be transferred over so patients don't need to complete a new Medical History.

If you are using a form type that is not Default or Advanced prior to Patient Experience, patients will need to complete a new Medical History.

The medical history form is set from any patient's medical history screen, via the spanner.

Patients can select only 'YES' answers if they desire. They do not have to select Yes or No for every question.
