Patient Portal Health Screening (COVID-19) Form
The Health Screening form enables you to collect important information about a patient's state of health, and was introduced specifically in response to the Coronavirus outbreak of COVID-19.
This Health Screening pre-appointment questionnaire collects the relevant patient information, saving it in EXACT, and triggering medical alerts where required.
Health Screening Form Questions
Forms present different questions to patients to meet regional nuances, for example; United Kingdom and New Zealand.
⚠️Note: Patients must answer YES or NO to every question and sign the form to save it.
Enabling and Disabling the Health Screening Form
⚠️Note: Available from EXACT 13.620.
You can stop sending this form to your patients with the following instructions;
- Go to Configure - Patient Experience.
- You may require EXACT administrator rights to complete these instructions.
- Untick the Enable Health Screening form option.
Completing the Health Screening Form
- The default date range for form completion is 14 days, this can be re-configured if required. Forms completed within the 14-day time frame are considered valid.
💡Tip: Contact EXACT Support if you would to adjust this value. - All completed forms are saved in each patient record in the Contacts tab.
- The relevant medical alert on the medical tab will be ticked automatically if a patient answers yes to any question.
- On the default form, answering yes to any of the questions will result in an infectious alert.
- On the long form, answering yes to the first 5 questions will result in an infectious alert. Answering yes to the last 6 questions will result in an alert, which indicates that they are potentially vulnerable.
- A pop-up medical note will be added on the medical tab informing that the patient is potentially infectious, potentially vulnerable, or both.
- On the arrivals tab reception can see the patient’s medical alert for appointments today and in the future to manage appointments safely. The arrivals tab allows for scrolling through future days easily to review future appointments as well.
- The heart icon on the arrivals tab represents the health screening form. It will display green if there is a valid form, black if there is a form an outstanding form.
Additional Information
- Introduced to EXACT V13.144 for customers using Patient Portal.
- The form for the United Kingdom is based on guidance from the NHS as of 14th December 2021.
- Any patients who have already been sent a form before disabling the form being sent will still have this available in their Patient Portal.
