Sending Forms to Patients With Patient Portal

Send forms automatically to patients before their appointments with Patient Portal.
⚠️ Note: Forms can also be completed manually, see 🔗 Manually Request Patient Forms with Patient Portal
💡 Tip: We advise using automation to request forms from patients via Patient Portal, as this helps eliminate the replication of forms displayed for patients to complete and reduces confusion for patients.
Requesting Forms via Patient Portal
Once your Patient Portal automation is configured, the correct forms will be sent to patients, as per your configuration, alongside their automatic appointment reminders.
Access Reminder Templates
To check your appointment reminders are configured to send Patient Portal email and/or SMS templates.
- Go to Configure - Automatic Appointment Messages.
- Select the Reminders tab.
- Confirm the templates used in your reminder setup use your Patient Portal templates.

💡Tip: The two main things to check are:
- The email or SMS templates used are Patient Portal specific reminders, these are usually named PORTAL REMINDER.
- In the Patient Has section, the correct Send Message combination of 1, 2, and 3 are ticked to send messages automatically.
Which Forms Will be Requested?
EXACT will send forms as per your practice configuration, with the configuration being different for the various forms, e.g. Medical History, Patient Details, etc.
Access this configuration via Configure - Appointment Book - Arrival Options.
Patient Details
This form is available to patients every time patient details are requested or any other form is due to be completed, in case their personal or contact information has changed.
Medical History
In the Arrival Options section click on Medical History.
The Alert if Medical History setting will determine how often this will be requested.
The number in the months overdue setting will determine how long it will be until a new medical history is requested from a patient since the last successfully completed medical history. For example; if this is set to 6 months, and a patient completes their medical history in April, this will not be requested again for 6 months.
💡Tip: When Alert if Medical History is set to 0, medical history forms will not be sent to patients.
Health Screening/Covid
This will be sent if the patient has not completed a health screening form in the last 14 days and the appointment is less than 14 days away.
⚠️Note: 14 is the default value, this can be changed by the EXACT Support team.
🔗 Patient Portal Health Screening (COVID-19) Form
Oral Health Survey
In the Arrival Option section click on Oral Health Survey.
Use the settings options on the prompt to set how often these are requested, with settings to configure which age patients to send the form to, and set the appointment provider type.
💡Tip: Disable the Oral Health Survey by removing the tick from the top left.
Contact Consent
In the Arrival Options you can enable/disable this under the Contact Consent tab by removing the green tick from the check box.
⚠️Note: This text will be used for Patient Portal and CliniPad.
💡Tip: Contact Consent will be sent for any patient who has not already consented to being sent marketing material.
Terms and Conditions
In the Arrival Options you can enable/disable this under the Terms and Conditions tab by removing the green tick from the check box.
The text displayed in the Edit Terms and Conditions box will be presented to patients as your terms and conditions.
You can set whether this is sent to new patients, patients who have not filled this out before or patients who have not filled it in since a certain date.
⚠️Note: This text will be used for Patient Portal and CliniPad.
FP17PR/FP17PRW
In the Arrival Options you can choose which patients are sent FP17PR or FP17PRW forms using the drop down option on this window.
⚠️Note: NHS will only send to patients if they have an NHS payor set on their patient record, set this to Always to send to everyone, regardless of their configured payor.
💡Tip: Set this option to Never, to stop sending NHS forms to patients
Form Frequency
The frequency at which some forms are requested is configured within your Appointment Arrival Options settings.
