When using Clinipad, patients can digitally complete their medical history forms on a tablet and have this imported directly into their patient record.

Clinipad Medical History Forms 

Clinipad requires you to use a pre-configured medical history form, there are three options to select from.

• Simple, consisting of a series of basic questions to keep things simple.
• Default, the same as simple, with some additional symptoms and habits to collate additional information regarding smoking, alcohol, fizzy and recreational drugs.
• Advanced, contains a full list of options with improved selection options.

⚠️Please Note: If you change your default template, all patients will be required to complete a medical history the next time they attend your practice, regardless of whether they have recently completed a form.

Manually Sending Medical History Forms to a Clinipad

1. In the patient record.
2. Click the Medical tab.
3. Select the Update Medical History using Clinipad option.
   • Select the required tablet using the dropdown list.
     
4. Once your patient has completed their forms, return the Clinipad session by clicking the down-arrow next to the Clinipad icon in your workspace, along the top of your system, and selecting the required device.

5. The medical history for the patient will now be updated, along with a version of the signed form.
   • You can find this information in the patient medical tab.
